Credentialing and Background Screening Release

Some of the clients and facilities we conduct business with will request copies of the caregivers credentials, background screen result, these may include but not limited to professional license, certificate, physician statement and proof of eligibility to work in the US.

Please sign below permitting Care & Beyond Nurse Registry to provide these credentials upon the request of the client or facilities.

We must comply by the policies and wished of our clients and each facility.
Failure to sign below may result in you not being referred to certain clients or facilities.

Hereby authorize Care & Beyond Nurse Registry to release information pertinent to my referral as requested by Clients and or Facilities.
