Independent Contractor/Caregiver Registration Requirements All Documents must be Originals
Proof (s) of eligibility to work in the United States is required for registration.
- Social Security Card, FL. Driver License/ID, US Passport, Permanent Resident Card, Work Permit or Birth Certificate.
Valid Florida Professional License/Certificate:
- C.N.A, LPN, RN
- HHA Certificate with at least 40 hours of training from a professional vocational technical school or licensed non-public education school.
Level 2 Background Screening with ‘eligible’ result.
- AHCA’s ORI (Original Agency Identifier) EAHCA020Z
Physical Examination from:
- A MD, Physician’s Assistance or an advanced registered nurse practitioner under the supervision of a licensed Physician dated within the last six months that Caregiver is free from communicable deceases.
Current CPR Card:
From an instructor who is approved to provide training by the America Hearth Association or the America Red Cross.

Continuous Education Course:
HIV/AIDS – 4 hours Original/Updates 2 hours
Domestic Violence – 2 hours (all caregivers)
Alzheimer’s – 2 hours (all caregivers)
Assisting with Medication – 2 hours for 40hour HHA
Medical Errors – 2 hours (RN LPN)
