Re: Service Hours/Time Sheet Log

This notice is to inform all Caregivers that all scheduled assignments with specific hours must be completed.

If you are scheduled to work for 10 hours per day you are expected to be with your client for those 10 hours unless you are authorized by your client or family member to perform services on their behalf.

Caregivers must contact the Care & Beyond Nurse registry if:

• You are running late.
• Your Client has changed your service hours.
• You have to leave your Client because of an emergency.

Requesting payment for service that was not performed is considered fraud. If you are suspected of exploitation, abuse or neglect the affected party has the option to file a complaint with DCF or AHCA which may result in the suspension of your licenses and or certification.

By signing your timesheet/service log you are certifying that you have completed the scheduled assignment in its entirety.
